Does anybody have a copy (blank or otherwise) of these.
Maybe I filled one of these 3 years ago or something, but pretty much these days I ignore them. However, I have need of one now to be appropriated for different purposes and I can't find one even in my hotmail 
So, even though they are regarded as lame, if somebody has one (or wants to remain anonymous PM me to save ridiclue  ) I would appreciate that so much thanks heaps.. and yes, I did google

your trying to use the dodgy forwarded get to know you email so i can get a chick your hitting on to unwittingly give you a heap of info you can use.... lol. a mate of mine did this sent to the chick and all her friends so it didnt look sus. worked a treat.
and no i dont ahve one.

I never used to do any of those surveys.. they can be kinda long and ghey at times "what did you have for dinner last night" "whats the name of every single pet you have ever owned" and even ghey-er than that shit like " when was the last time you cried.." i mean wtf?!
Who remebers that shit, let alone fills it in on a random internet survey ??

Technically yes.. but seeing as i dont ever fill them out it wouldnt have worked for me
Half the time they are sent filled out from a mate so you have to erase their stuff then fill in your own...
i dont think my attention span is that long.... ^-^
