I've had 2 done, one on a Mk II Supra(which I didn't end up getting, the inspection was right, car woulda cost me more money to fix than to buy..). I got another one done on my current TA23, was really helpful, pointed out that my front suspension had seized(explained the shite ride over bumps, so I got that fixed), they give you a full rundown on the car, any problems they find they try to give an estimate of repair costs. My car's got a K&N filter, big exhaust and a few other goodies and he didn't say a word about engineering, but then again neither does my mech when I take it for a rego either
