I bet my money its 1 of two things. Its either only running water which in a old car like that is likely and it turns to mud and blocks the radiator. Very common as i see it every day or he has let it get hot to many times and has cooked the head. Also see that very often. Get someone to dump the water and high pressure flush it and do a head test on it before buying if possible. Or just offer him a smaller amount to leave yaself some money for the obvious repairs it will need. But start with a radiator cleanout and high pressure flush.
