sounds like noise is related to engine speed, not output shaft speed? so eliminate everything south of the input shaft bearing.
was there much wear on the pressure plate fingers (from the throwout bearing?)
maybe clutch is in wrong way and the centre is rubbing against or near the spigot bearing - might explain the noise at clutch disengaged and when cold?
have you had a look at the oil in the gearbox?
is starter gears full disengaging from ring gear on flysheel?
or is could be mechanics horror story - like no oil the gearbox, or no thrust washer on the input shaft bearing?
ora dumb thing done by owner (like me who degreased gearbox before reinstalling, forgetting that the throwout bearing was in the bellhousing so i successfully degreased it and had a noisy clutch from day one...
can you get it up on a stand, run in gear and get under to listen for the noise (i know you shouldn't run engines in gear on a hoist ... but what the heck...)
