I really dont think you have thought it through, your front discs use a hub that bolts onto the discs, which means the disc sits on the stub axel. Every rear disc brake setup i seen uses a floating disc with the hub being part if the axel. Anything is possible but the fabrication costs would be astronomical. It would be cheaper to buy a diff (e.g borg warner) with a disc brake end setup, and shorten that and have the brakets welded on and you would prob still walk out with change in ur pocket over the other option.
