Check for air leaks in the system, and yes blockages anywhere in the system can also cause those symptoms. I had a similar thing happen recently when one of the hoses got a pinhole leak and started drawing air in; you'll know this is happening if you see little bubbles rising up in the fluid reservoir when the engine is running.
