It sounds like the discs are worn and thus warping when they get hot. Check the thickness of the discs and compare to the minimum thickness specs of your discs (you can look it up on DBA's web site if you're not sure).
As above check the condition of your brake pads, and even if they're in good condition throw them out and get some decent pads!
