I thought this too once, but 4 second hand gearboxes later and still not have one that was acceptable (all warranty swaps). I will be rebuilding mine this time round.
It's pot luck really whether you get a good one or not, with the 4 T-50's I got from one wrecker with each one I was very unlucky, and they were all fitted to the car and driven each time!! Then removed and replaced, what a fu(ken waste of time!! I got damn quick at gearbox swaps though.
The W58 I have now has a noisy bearing rattle on light load/deceleration in 4th, which it had from the time it was purchased  . I will be fixing this and not replacing this time round 
I guess it comes down to: "Do you feel lucky??"
As for the 5th gear problem:
Check the gearstick is engaging 5th i.e. the gearstick is able to move far enough without obstruction (I am sure you have already covered this one)
As someone has already mentioned, check the gate and that there is a ball and spring engagin the gate (sometimes these little things go missing).
