Can occur when bolting a manual gearbox up and the box input shaft hasn't aligned properly, dicky or incorrect spigot or input shaft is too long. You are effectively pushing the crank forward as you tighten the bellhousing bolts up.(a good one to watch when doing a 3SGE RWD conversion) Bummer.

I'd be suprised if most arn't tapering across the flats.
