1: W58 or R154 for 5 speeds, V160 for 6 speed

3: No, but you would probably need to trick it into thinking it was constantly in Nuetral.
4: Think about it, how else will you get drive from the motor to the box - cos I'm sure you're fully (Fooly?!?) aware manual flywheels look nothing like auto flexplates?!!!
5: 2-6Kish, dependant on the type of box, condition of parts, amount of labour, ...
