The same thing happened to mine quite a while ago now, but I was lucky enough (I guess) to have the key sheared & also the threads on the bolt completely work away, so the crank itself was very much reusable. The pulley was pretty rooted too.
I've heard that the original single piece bolt (1 piece bolt & big washer) had something to do with this problem, & toyota changed the design to a 2 piece bolt & seperate washer in an attempt to rectify the prob. If you buy a new bolt from toyota it will be of the newer 2 piece design. The washer doesn't come off the bolt but it is loose & free to rotate. How this would affect things I have no idea. Maybe something to do with preloading the key at the assembly stage....

Probably the best (and of course hardest/most expensive) solution to the problem is to get rid of the stupid tiny little half moon key & key way slot & have the keyway in your crank machined out to take a proper full length key, which would put up with alot more stress without failing. It does of course mean pulling the crank out, which wouldn't be worth doing just for that reason I reckon, but if you're ever getting a 4AGE rebuilt it's definitely worth having done, and would only add around $80 to your total rebuild costs. It seemed a rather insignificant cost to me when I was getting my current motor rebuilt for 4AGTE use.
