I have a '90 4Runner, I'm of the opinion from what I've heard with trucks and 4Runners... and from my own experience that the speedo cables are prone to fail.
I've got a '76 Corona MkII with approx 500,000 kms and I've never had to think about the speedo cable. I've already replace the speedo cable on my 4Runner with a brand new OEM one once and had to service it again a few years later. When they start howling, buzzing, squealing it can be the most disconcerting noise because you're more less stunned and in disbelief that an otherwise healthy vehicle could make such a noise. Anyway, you mention that it doesn't seem to correlate with revs so to me that sounds like it isn't related to loose exhaust or mounts. Sounds more like a speed dependant thing like the mud flags example. But since it's a truck... I'm thinking speedo cable.
It's a pain to disconnect and lube, but you might get lucky trying to service it. If it's too far gone, you'd probably have to replace. Years ago I paid $90 cdn for a new Toyota cable and replaced it myself so it wasn't too hard to cope...
