Think of a water to air IC as two radiators, one which is encapsulated (heat exchanger) and one not.
The water flows around the two radiators via a moderately sized water pump.
The heat exchanger works in reverse to the way a normal radiator works, the water actually cools the charge air instead of the air cooling the water.
The main radiator is then cooled by air in a normal fassion.
