Leaking kitchen tap. How do i fix?

Hey my kitchen tap is leaking. Im desperate for advice on how to undo the damn thing to change the seals/washers.
It sorta looks like this

I wanna try DIY before i resort to calling a plumber in.

Dude,
Unbolt it from the Sink. (Turn the Water Main off 1st)
Then take it to Tradelink plumbing supplies (NSW, branches everywhere)
They should be able to get something.
Though buy the look of that picture it's probably Ceramic Disc( which means Toss it and buy a new one)

Thanks ppl.
I removed the hot and cold coloured caps to reveal a screw.
Undid those and the handles come off.
Just gotta wait till morning to get new seals.
