Genuine from Toyota they aren't $350, they are $213.
Apparently there is a place down south that sells a kit to place a very similar one in an 18RG with only small modifications to the tensioner for $48. Apparently they work fine.

I found a good 18RG quite cheap, of course it didn't come with the tensioner.
