It is a well recognised fact that synthetic underwear increases moisture in the nether regions. This usually increases the incidence of fungal growth (Candida, the most common infection - bacterial overgrowth is unusual because of the rather rich and complex bacterial makeup down there). As well as the use of antifungal creams (Canestan and the like), it is generally recommended that the patient wear natural fibre underwear or forego them altogether. Natural fibre (eg cotton) allows better ventilation and therefore drier conditions such that fungal overgrowth is discouraged.
Urinary tract infections, which are becterial in nature, are not related to underwear (or lack thereof).
