need a 5th gear for supra turbo R154 gearbox (the small gear on the output shaft)
anyone have bits of a broken box around?

Try Nick ('wastegate'). He had a broken R154 for sale a little while ago. Otherwise, Rod Hadfield at The Rod Shop may have some. If not, you could get a gear made by these guys.
