i do not know which vehicle it came off originally. I am lead to believe that only the us spec trucks recieved them thou.

as far as fitment it bolts directly to the head and the water jackets are a direct bolt on. you just need to sort out the oil lines and intake piping. It helps to if it is already efi, its alot less work again.

$400 is minimum. turbo and manifold are in good condition. i forgot to take my camera to work today so no picks yet.
